First Connections
This project provides home visiting services and group educational classes to at-risk pregnant and postpartum women. Home visits conducted prenatally and postpartum provide participants with parent education on topics such as prenatal development, labor and delivery, breastfeeding and infant development. At-risk mothers are also provided case coordination services designed to address risk factors and improve birth outcomes and bonding.
