Children’s Mercy Hospital | Promise 1000
The Promise 1000 – Home Visiting Collective partners with local home visiting agencies on a common mission to implement an innovative, sustainable, collaborative system of evidence-based home visiting services for pregnant women, young children, and their families to (1) Improve maternal and child health & well-being, (2) Promote child development and school readiness, and (3) Increase resilience and safety of participating families.

Columbia/Boone County Department of Public Health and Human Services | Brighter Beginnings
Brighter Beginnings focuses on increasing access to individualized home visiting, parent education and pregnancy support service for pregnant people and families with young children in Boone County with an emphasis on improving birth outcomes and school readiness, as well as addressing disparities in these outcomes. Brighter Beginnings prioritizes resources to the most vulnerable families in Boone County through the Coordinated Referral and Intake System, which connects families to services based on their needs and eligibility.

Community Partnership of the Ozarks | Family Focused Network
Family Focused Network (FFN) – Collective Impact (CI) project is working towards building a stronger foundation for home visiting services to provide support and services to all families with young children. This collective impact project has 4 main goals: market the benefits of home visiting to the community and professionals in our focus area, support a Coordinated Referral and Intake System (CRIS), support data collection related to home visiting services, and support home visiting providers with needed trainings and certifications.

Generate Health STL | St. Louis Home Visiting Collaborative
This program seeks to expand home visiting to serve more families in need and contribute to elminating racial disparities in maternal, child and family outcomes by building the necessary infrastructure to support collaboration and coordination among provider organizations and ensuring that provider organizations are prepared to meaningfully and respectfully enage with families in our region to provide supportive and safe environments for working together.

Whole Kids Outreach, Inc. | Southeast Home Visiting Collaborative
This program is the backbone agency for Southeast Missouri’s Home Visiting Collective Impact Project. It will use its existing network of home visiting agencies and resources as a starting point to establish strategies to reach and prioritize high-risk populations. Special efforts will be made to target those who are likely to provide services to families in high-poverty areas by identifying and establishing relationships with all agencies who either provide services through an evidence-based home visiting program or who have the potential to do so. WKO will also work with regional service providers to generate referrals by promoting the benefits of home visiting and the coordinated referral and intake system (CRIS).
