The following agencies and programs were awarded Children’s Trust Fund (CTF) Home Visiting Grants for state fiscal years 2024-2027 (July 1, 2023 – June 30, 2027).
Family Forward | Healthy Families America
This program supports families by providing home visiting services that aim to cultivate and strengthen nurturing parent-child relationships, promoting healthy childhood growth and development, and enhance family function by reducing risk and building protective factors.

Nurses for Newborns | Nurses for Newborns
This program seeks to increase the number of mothers & infants receiving infant prenatal care, appropriate primary preventative medical care, and the number of infants who receive all required immunizations. Overall, this program seeks to ensure caregivers who complete the program have the parenting skills needed to safely care for their infant and to prevent abuse or neglect.

Nurture KC – Promise 1000 HV Collective | Promise 1000 Home Visiting Collective
Funding for this program collectively supports five home visiting agencies serving families in Jackson and Buchanan counties, with a focus on preventing child abuse and promoting parental strengths through the protective factors and positive parental interactions and relationships with children. It also aims to prevent childhood injuries due to neglect by improving home safety and safe sleep practices within Promise 1000 families.

Springfield-Green County Health Department | Family Connects
Family Connects Springfield-Greene County is a universal, evidence-based newborn home visiting program focused on community alignment and engagement, offered at no cost to all families with a newborn in Greene County. This a voluntary program where a family is paired with a registered nurse and will receive a health assessment for the mother and infant, screenings including mental health and substance use, education on caregiving, breastfeeding support, safe sleep, as well as a robust plan for referrals to community services.

Whole Kids Outreach, Inc. | Healthy Families America and Nurses for Newborns
Through these programs, Whole Kids Outreach, Inc. aims to improve family well-being and decrease instances of child abuse and neglect for present and future generations, and increase the amount of home visiting services provided to families living in high-poverty zip codes within its service area.
