
CTF is excited to partner with Home State Health’s Show Me Healthy Kids to connect pregnant foster youth to necessary home visiting services. Show Me Healthy Kids (SMHK) is a health plan that coordinates health care services for children and youth in foster care, extended foster care, adoption support, and young adult alumni of the foster care program.
Supporting youth in care through their pregnancy and connecting them to home visiting services will improve outcomes in health, safety, education, and economic mobility for Missouri’s most vulnerable mothers and babies.
Providers can make a referral for foster youth through CTF’s Coordinated Referral & Intake System (CRIS) or by scanning the QR code in the downloadable flier below.
