      CTF believes in the preventative outcomes consistently demonstrated by high-quality, evidence-based home visiting services. Such services have consistently resulted in positive effects on children’s health, safety, and educational outcomes.

Statewide Home Visiting Outcomes Rate Card

The Statewide Outcomes Rate Card provides new, incremental incentive payments to participating home visiting providers based on the achievement of certain outcomes. The selected metrics are within these four priority realms:

Home Visiting Participation
Prenatal Enrollment
Child & Family Wellbeing
Healthy Birth Weight

We believe these metrics will:

Directly improve the lives of Missouri’s most vulnerable families
Equitably scale home visiting services for those who need it most
Contribute to the larger evidence base that supports home visiting and its ability to generate social and fiscal benefits long-term
