What is the CRIS Link

The CRIS Link makes accessing early childhood (prenatal to five) home visiting services easy for anyone in Missouri who needs it. It is a one-stop shop for anyone to request early childhood home visiting in Missouri through one of five Regional CRIS Networks, depending on the needs and location of each family. Whether you are a home visiting provider, a medical professional, an educator, or a caregiver seeking services, you can request home visiting for a family using the CRIS Link.

Select the family’s county using the CRIS Link below to find their Regional CRIS Network and referral form. The referral form is what you fill out to request home visiting services. Once that form is submitted, the Regional CRIS Network will assign the family to a home visiting program based on the family’s needs and eligibility, and the program’s capacity.

The assigned home visiting agency will then contact the family to tell them more about their program and offer services.

  • Our Home Visiting Collective Impact Partners manage our five Regional CRIS Networks within their specific regions. Contact your Region CRIS Network’s CRIS Coordinator to learn more.
  • In an effort to connect families to home visiting services as early as possible, the Missouri Children’s Trust Fund has partnered with Healthy Blue and Home State Health’s Show-Me Healthy Kids to automatically refer pregnant women receiving Missouri Medicaid and foster youth to receive home visiting services with consent.
